ABSTRACT: One of the normal developmental tasks of adolescence is to separate from family and to struggle towards a sense of autonomy and independence. This developmental process can be markedly interrupted when a young person uses drugs. In this proceeding from the ISST International Congress, 2017, the author addresses the cyclical destructive impulses inherent in clients who are abusing drugs. For substance users, the circularity of time is marked by drug-taking and drug-craving, representing a beat of pleasure followed by the feeling of pain. The clients’ interrupted therapies, plans, and lives rattled by constant relapses seem to make every action in vain. The ultimate goal of those who use drugs is to cancel their own lives. Images of this total destruction may, at times, appear in the sand tray, but such images may also be signs of the necessary ending of one phase in order to gain access to a new period. As a metaphor for this struggle for autonomy and independence, the Tuscan folk tale of “Piccirillino,” is used to illustrate that, by accepting a descent into putrid matter, one can nonetheless grow and emerge transformed. The author presents three clinical examples of patients who are trapped in a web of pathological dependencies, where the process of growth runs aground in the intricacies of a dark and ambivalent desire. Each client works through a descent and emerges in a unique manner.
